UnitedHealthcare acupuncture coverage - know your plan
Coverage Overview
UnitedHealthcare (UHC) covers acupuncture under most of its commercial plans, including employer-sponsored PPO, HMO, and Choice Plus plans. UHC has expanded its acupuncture benefits in recent years, particularly for chronic pain conditions, as part of its broader focus on reducing opioid use. Nature Acupuncture & Herbs is in-network with UnitedHealthcare.
UnitedHealthcare acupuncture benefits typically include 10-20 visits per year for covered conditions. Coverage applies when treatment is medically necessary and provided by a licensed acupuncturist enrolled in the UHC network. UHC PPO plans (such as Choice and Choice Plus) generally allow both in-network and out-of-network acupuncture. Visits are subject to your plan's deductible and specialist copay.
What's Typically Covered
- ◈10-20 visits per year on most commercial plans
- ◈Chronic low back pain, headaches, nausea covered
- ◈PPO plans allow in-network and out-of-network
- ◈May be covered under complementary/alternative medicine benefit
- ◈Subject to deductible and specialist copay
- ◈UHC Optum supplement programs available on select plans
What UnitedHealthcare Publishes
UnitedHealthcare publishes a commercial reimbursement policy for acupuncture. It requires the service to be billed in 15-minute increments of face-to-face contact with the patient rather than by how long the needles stay in, and allows only one initial code, 97810 or 97813, per day. It sets maximum units per date of service of one for 97810, two for 97811, one for 97813 and two for 97814. The policy governs how a visit is billed; how many visits you get is set by your own plan.

How to Check Your Own UnitedHealthcare Benefit
Plan documents are written for the plan, not for you. Two people with the same carrier on their card can have completely different acupuncture benefits, because the employer chooses the design. Anything below describes what the carrier publishes; only a call against your own member ID tells you what you have.
Before you call
- ◈Your member ID and group number, both on the front of your card
- ◈The member services phone number from the back of your card
- ◈Your date of birth and the subscriber name, if the plan is not in your name
- ◈A rough description of what you want treated, and how long you have had it
What to ask, word for word
- 1.“Is acupuncture a covered benefit on my plan, and does it need prior authorization or a referral?”
- 2.“Are CPT codes 97810, 97811, 97813 and 97814 covered, and how many units per visit?”
- 3.“How many acupuncture visits am I allowed per year, and how many have I used?”
- 4.“What is my copay or coinsurance for acupuncture, and does my deductible apply first?”
- 5.“Is a licensed acupuncturist (L.Ac.) an eligible provider type, or must it be an MD or DC?”
- 6.“Does coverage depend on the diagnosis, and which conditions are eligible?”
If it is not covered: If acupuncture is not covered on your plan, we will tell you before you book rather than after you have been treated, and we can talk through self-pay options. We also accept FSA and HSA cards.

UnitedHealthcare Acupuncture Coverage FAQ
Does UnitedHealthcare cover acupuncture?
Yes. Most UnitedHealthcare commercial plans cover acupuncture as a medically necessary service. Covered conditions typically include chronic low back pain, headaches and migraines, nausea, and osteoarthritis. Coverage is provided through either the main medical benefit or a complementary and alternative medicine (CAM) rider, depending on your plan.
How many acupuncture visits does UHC cover per year?
Most UnitedHealthcare plans cover between 10 and 20 acupuncture visits per year. Some plans include acupuncture under a combined CAM visit limit that also applies to chiropractic and massage therapy. We recommend confirming the visit limit and whether it is shared with other CAM services before booking.
Is acupuncture covered under a UHC employer plan?
Acupuncture coverage under employer-sponsored UHC plans depends on the benefits package your employer selected. Not all employer plans include an acupuncture benefit even if UHC offers it - it may be an optional add-on. Check your Summary of Benefits and Coverage document or call the member services number on your UHC card to confirm.
How do I find a UnitedHealthcare-covered acupuncturist?
You can search UHC's provider directory at myuhc.com, or call the member services number on the back of your card and ask for acupuncturists in your area. You can also contact our office directly - we'll verify whether your specific UHC plan covers our providers before your first appointment.
