Which UnitedHealthcare plans cover acupuncture?
UHC's Choice Plus, Options PPO, Core, Navigate, and Select plans all commonly include acupuncture as a covered benefit. HMO plans may require a referral from a primary care physician, while PPO and Open Access plans generally do not. Optum-administered plans use the same network as UHC commercial plans, so in-network status applies across both.

What does UHC cover?
UHC commercial plans typically cover acupuncture for musculoskeletal conditions and chronic pain -- back and neck pain, joint pain, headaches, and osteoarthritis are consistently included. Cosmetic acupuncture is excluded under all UHC plans. Standalone herbal medicine consultations are not covered and are billed as cash-pay services.
Visit limits
Most UHC commercial plans allow 20 acupuncture visits per calendar year. Some employer groups negotiate higher limits -- 30 or more visits per year -- particularly in industries where employers compete for talent on benefits. If you have a chronic condition that requires an extended course of treatment, additional visits beyond the standard limit are sometimes available with preauthorization and documentation of medical necessity.
Cost sharing with UnitedHealthcare
Under most UHC plans, acupuncture visits are subject to your annual deductible and then apply a specialist copay or coinsurance rate. Specialist copays through UHC typically run $40 to $75 depending on the plan, with coinsurance plans usually falling between 20% and 30% of the contracted in-network rate after deductible is met.
In-network contracted rates are always lower than out-of-network rates. Seeing an out-of-network acupuncturist with a UHC PPO plan typically means higher costs and the burden of filing your own claims. In-network care eliminates both of those disadvantages.
UHC Medicare Advantage and acupuncture
If you have a UnitedHealthcare Medicare Advantage plan -- including AARP-branded UHC plans -- acupuncture coverage follows the 2020 CMS mandate. Medicare Advantage plans are required to cover up to 12 acupuncture visits per year for chronic low back pain, with an additional 8 visits available if documented improvement is shown (up to 20 total per year). This benefit applies to medically diagnosed chronic low back pain only.
Some UHC Medicare Advantage plans have expanded acupuncture coverage beyond the federal baseline. Your plan's Evidence of Coverage document will specify what is included.
