When a patient is doubled over with period cramps, I am not going to pretend a few pressure points replace real treatment or, when it is needed, a doctor. But acupressure is genuinely useful for menstrual cramps as self-care between visits, it is free, and you can do it curled up on your own couch, so it is worth knowing a few of the points I actually recommend.
A caution before the points
First the honest part. Severe, worsening, or unusual period pain is not something to simply press away; pain that is getting worse over time, or that comes with very heavy bleeding, can point to endometriosis, fibroids, or other conditions that deserve a proper evaluation. So use these for ordinary cramps, and if yours are extreme or changing, see a physician. With that said, here is what tends to help.

Spleen 6, Sanyinjiao, is the point I mention most for menstrual pain. Find it about four finger-widths up from the inner ankle bone, just behind the shin bone, and press firmly; it is often tender around your period, which is how you know you are in the right place. It has a long history for menstrual and pelvic complaints, and slow, steady pressure for a minute or two on each leg can take the edge off cramping. One important caveat: SP6 is traditionally avoided in pregnancy, so skip it if there is any chance you are pregnant.
LV3 and LI4 for the tension
Liver 3, Taichong, sits on the top of the foot in the web between the big toe and the second toe, back toward the ankle. It is my go-to for the stuck, irritable, tense quality that so often rides along with cramps, the premenstrual clenched-jaw feeling. Pair it with Large Intestine 4, Hegu, in the web between thumb and index finger, and in Chinese medicine you have opened what we call the Four Gates, a combination for moving stagnation through the whole body. LI4 is powerful for pain generally, but like SP6 it is avoided in pregnancy.
A warm point on the belly
Lower on the abdomen, a few inches below the navel, is the region of points like Ren 4 and Ren 6 that relate to the uterus and lower warmer. You do not need to be precise; gentle, warm pressure or a heating pad over that area, combined with the leg and foot points, is a comforting, low-tech way to ease cramping.

Think of acupressure as first aid, not a treatment plan.
